Acknowledgement COVID-19 Form

 Participating in any recreation programs/clinics/camps, organized sports,  or usage of any Township outdoor facility must complete and sign a waiver form.  All forms must be filled out completely and forwarded to the Recreation Department five days before the event/activity.
